So, Belasco (the villain cutting the cake) - is doing so because it's... what's the right word? A reference? A nod?... to when you cut the cake. Ka-Zar (the guy in the tux with the knife) and Shanna (the woman in the bridal gown) are supposed to be references to being a wedding decoration toppers (that sites on the top of the cake - sort of like
this). So she's not head butting the cake (if you mean Shanna), she's jumping off the cake.
Er, I think there was some misunderstanding here. For a start, I can see that the villain (Belasco) is cutting a wedding cake; I was wondering why the sword (other than being a fairly obvious way to show how savage/heartless/etc. he is).
OK, so I can see that Ka-Zar and Shanna are (or were) presumably getting married, and Belasco is interrupting the wedding in traditional comic book style. So far, so good.

What I
am puzzled about is the person in the bottom right, just below Shanna. He has blond hair, and is wearing a kind of bronze armlets and a bright blue cape, and it looks like he is "headbutting" the cake.
Or perhaps I'm completely mistaken and "he" is part of Shanna? In that case, his cape is part of Shanna's dress, his arm is Shanna's leg, and his blond hair is ... not something I'd like to speculate about!

(Unless Shanna has a horse's tail?)
Also, upon reviewing the picture, I'm surprised to see that Shanna's wedding dress has armoured sleeves. Was she expecting trouble, maybe?
